Patients with a history of heart failure have complex medical needs and often require frequent follow-up by cardiac specialists. The program at The Medical Group is designed to improve and maintain patient health by assuring optimal medical therapy. The program is run as a collaborative practice with the cardiologists and the nurse practitioner, and is targeted at managing heart failure as well as other related health conditions. Patients under the care of outside cardiologists may also join the program, and records and progress will be shared regularly.
